Family Health Teams are playing an important role in improving quality of care and access to primary health services across the Province. FHTs are an initiative launched by the Ontario Ministry of Health. The FHT concept involves bringing physicians and other health professionals together to provide high quality primary care.

Corporate Structure

The model employed is of a physician led Board. A seven member Executive Committee meets regularly to advise and direct the FHT.  From conception in 2006, to inauguration in 2007 and expansion in 2008 it has been a constant learning process.

Serving the Markham area for over 30 years

Family Health Teams are playing an important role in improving quality of care and access to primary health services across the Province.

FHTs are an initiative launched by the Ontario Ministry of Health. The FHT concept involves bringing physicians and other health professionals together to provide high quality primary care.

The health team concept, in Markham, dates back to the early 1990s when Markham Family Physicians, MSH CEO Dr Jim MacLean and the Department of Family Medicine formulated a concept featuring interdisciplinary care available to our patients. The work, a little ahead of its time, was held in waiting for the correct medical and political climate. Changes in health care in 2005/2006 have now allowed the ideas to become reality.

Markham Family Health Team was built on the determination of Markham Family Physicians, The Markham Stouffville Hospital Family Medicine Department and local Family Physicians Drs. Esterbauer and Um. Dr William Newton provided the necessary leadership and patient guidance while Drs Tom Filosa, Jeff Weisberger, Ron Esterbauer and Stephen McLaren formed the MFHT Executive Committee.

The family physicians involved have a long and healthy relationship with Markham Stouffville Hospital and the local specialist community. Markham FHT will continue these relationships and entrench the positive attributes of the relationship that serve our patients so well.
