Markham Family Physicians experienced a cybersecurity incident in December 2025, during which certain information was accessed and exfiltrated from our systems. Following the incident, we notified all potentially affected patients directly by letter or email. If you previously received a notice from us regarding this incident, please read this update carefully.

If you did not receive a letter or email from us regarding this incident, then your personal information and personal health information was not affected and this update does not apply to you.

We want to reassure our affected patients that we took this situation very seriously from the outset. With the assistance of specialized experts, we conducted a thorough forensic investigation and implemented additional safeguards to help prevent a similar incident from occurring in the future.

Unfortunately, our investigation has confirmed that some of the information taken from our systems has been published on the dark web. While we do not know exactly which specific information may have been involved, we cannot rule out the possibility that your personal information is among it. For this reason, and in the interest of transparency, we felt it was important to inform you.

The personal information and personal health information relating to you that may have been involved in the December incident remains limited to the information identified in the notice you received in February.

Our after-hours Urgent Care is available to all registered patients of the Markham Family Health Team. Urgent care hours are for appointments only. Walk-ins are not accepted.

In addition we also offer a “Doctor Available” dedicated for same day urgent daytime visits.  Please call 905 471-9999 to book this service if your regular provider is not available and you need urgent attention.
