Group Lifestyle Balance

Markham Family Health Team is pleased to offer you our Group Lifestyle Balance Program!

This program is designed for individuals who would like support and education on achieving weight reduction and increasing physical activity. The goals of the program are to lose 7% of your body weight and increase physical activity to 150 minutes per week.

Weekly topics include healthy eating, physical activity, problem solving, and more!

Do you Qualify?:

● 18 years of age or over

● Are interested in losing weight and reducing risk of type 2 diabetes and other chronic

diseases

● Can commit to attending most classes

This program focuses on intensive lifestyle intervention and is funded by the Ministry of Health, therefore has no direct cost to you.

Registration is now open for our November intake!

Start Date: Wednesday November 15th 2017 and will run weekly for 12 weeks (with breaks for holidays), followed by 4 biweekly maintenance sessions.

Offered at: 1:30pm at Cornell Public Library, Markham (3201 Bur Oak Ave.)

and 7pm at Bayview Hill Community Centre, Richmond Hill (114 Spadina Rd.)

Space is limited, and choice of which session you can attend will be based on a first-come, first-serve basis.
